Are you a practitioner looking for a change in role or to further your experience or development working with children and young people, working in the Community?

We are looking for motivated professionals with good knowledge and experience of working with children, young people and families. You should have the ability to work within a skill mixed team to deliver allocated components of the Healthy Child Programme 5-19 (25 for SEND), to develop positive relationships with children, young people and families which enable them to be engaged, participate, take responsibility and make informed choices and decisions.

Working as a team player under the delegation of a School Nurse your role will require you to work in partnership with schools, to promote and enhance the health and well-being of children and families, to deliver specialist targeted interventions and groups to meet the needs of children, young people and families identified as at risk of poor outcomes or with complex needs.

HCRG are working as an integrated partnership with Barnardos as The Essex Child and Family Wellbeing Service (ECFWS), delivering innovative and refreshing health and social care from pre-birth to 19 years services to Essex Families. We have developed our own unique approach where our children & young people and families are at the centre of everything we do. We are currently looking for practitioners to join our Healthy Family Teams to help us continue to improve things for the better.

On behalf of the NHS, Essex County Council and WECCG, HCRG Care Group and Barnardo’s are commissioned to provide a range of children, young people and family services throughout Essex (excluding Thurrock and Southend) that are free at the point of delivery.

The ECFWS focusses on ensuring that everyone in Essex has the best possible start in life through meeting the identified needs of children, young people and their families. 

Our services are offered across Essex from the Antenatal period up until a young person turns 19 (or 25 for those with Special Educational Needs).  We are located within the community delivering outcome-based support to families within their homes, schools or at one of our Family Hubs and/or Delivery Sites.

As a Children and Young People Practitioner you’ll be working as part of the ECFWS in the South of Essex where we deliver services across all mainstream schools, and you’ll join a team who are focused on clinical excellence and who have received a “Good” CQC rating.

You will be working within Pre-Birth to 19 Healthy Family Teams, consisting of Health Visitors, School Nurses, Healthy Family Support Practitioners, Healthy Family Support Assistants, Community Engagement team and Healthy Schools Engagement Workers, working within The Healthy Child Programme, supporting the delivery of a universal School Nurse service, supporting all aspects of a child/young person's health and wellbeing and supporting them to make positive life choices.

Main ResponsibilitiesAs a Children and Young People Practitioner (CYPP) you will be responsible to 

 


Work in collaboration with the School Nurses to meet the health needs of the children and young people within the community, helping children and young people to develop a knowledge of self-care, autonomy and decision making, including how to access health services


 


Deliver aspects of the Healthy Child Programme 5-19 pathway as delegated by the School Nurse
 
Provide Brief Intervention and Solution focused support in relation to a range of issues including emotional well-being, exploitation, sexual health, relationships, substance and alcohol misuse, risk taking and anti-social or offending behaviour, Healthy Lifestyle. These may take place in school, in a home setting or a Family Hub or Delivery Site.
 
Work in a multi-disciplinary framework with key partners, including the statutory and voluntary sector, in identifying and responding to young people and family needs.


 


Deliver targeted Health Promotion Campaigns, through the medium of School Drop In clinics, assemblies and community outreach.


 


Work with parents, carers and families to ensure that they are appropriately engaged in the support process and that they know how to access support for themselves and their children.


 


Assess and manage own workload under the supervision of the School Nurse


 


Contribute to the delivery of required Key Performance Indicators and Outcome Measures to measure the impact of commissioned service specifications
