As a Band 6 Staff Nurse in West Lancashire you will play an integral role within our Urgent Community Response Team. 

The Urgent Community Response Team aims to ensure crisis response care is available to all people within their homes or usual place of residence, including care/residential homes, within two hours.  

The service operates over a seven-day rota, 365 days a year and is operational between the hours of 8.00 am – 8.00 pm so we can offer flexible working patterns.

Main ResponsibilitiesAs a Staff Nurse you will be part of a multi-disciplinary team who will identify patient needs and coordinate intensive input in the patients home to prevent avoidable hospital admission and stabilise the patient.  You will work as part of the team under the direction of the Clinical Co-ordinator, you will provide high quality holistic care to patients and you will also work closely with primary care, NWAS, 111, Acute Visiting Service (AVS) secondary care, social care and third sector organisations. 
The Ideal CandidateYou will be a registered nurse with NMC registration and have experience undertaking complex assessments and the planning and evaluation of care packages. 

We have built organisation-wide committees in many specialist fields. You’ll have the option to join your clinical specialism, networking with professionals from across the country. We get together regularly, auditing and sharing standard methodology for innovation and service re-design. You’ll be a part of the pool of subject matter experts bringing together clinical and operational leads. You will be a part of quality improvements, research and development activity. You will also need to be confident using IT systems and be able to demonstrate working knowledge of Microsoft Office packages i.e. Outlook, Word, Excel.
