We’re looking for an organised and compassionate Patient Flow Coordinator to help ensure a smooth and supportive journey for patients and their families.In this key role, you will coordinate the patient pathway from the point of referral, working closely with families, doctors, ward teams and the discharge coordinators to ensure clear communication and seamless transitions throughout care.Key ResponsibilitiesCoordinate the patient journey from point of referralCommunicate effectively with patients, families and multidisciplinary teamsMaintain and track patient information using EMIS and ExcelSupport efficient admissions, transfers and dischargesEnsure every patient and family experience is as smooth and supportive as possibleUphold and reflect the values of the hospice in all interactionsWhat We’re Looking ForExcellent communication and organisational skillsIT literate, with experience using systems such as EMIS and ExcelAbility to manage multiple prioritiesA compassionate, patient-centred approachHoursMon- Saturday 7:00- 15:00 or 08:00- 16:00 as suitsMake a difference by helping every patient’s journey run smoothly, with dignity and care.
