To provide a high quality specialist mental health service which meets the individual needs of vulnerable people with complex needs.

Principle Accountabilities:

Assessing support needs writing, implementing monitoring and reviewing support plans and risk assessments to meet the individual needs of service users.

Co-ordinate and maintain the package of support including personally providing support and accessing and monitoring other resources, including:

Acting as a key provider of recovery based mental health support services
Advocating on behalf of service users with other agencies
Welfare benefits advice and budgeting skills
Accessing resources from statutory and voluntary agencies and wider community
Encouraging skills for living independently and successful maintenance of a tenancy
Enabling access to employment and training services
Assist tenants to gain access to move-on housing, draw-up and implement resettlement plans and provide follow-up support in conjunction with other agencies/staff.
Provide information and consult service users about services provided and actively encourage participation.

Implement procedures for selection and allocations, including:

Seeking referrals
Interviewing and assessing prospective service users

Liaise with Specialist Housing Management (SHM) around issues such as:

Rent arrears recovery
Voids control
Resolving conflict between tenants
Implementing housing management policies and procedures
Maximising tenant’s income
Ensuring health & safety requirements are met

Be proactive in identifying potential and actual repairs and need for improvements and adaptations, including:

Reporting repairs and liaising with SHM to ensure works are carried out to required standards
Making recommendations for the purchasing of furniture and equipment

To work effectively as a team member, including:

Sharing information
Taking on team wide responsibilities
Providing cover for other team members
Contributing to the development of the service by taking a lead role in a specific area of work e.g. service user involvement, health & safety
Contributing to duty cover arrangements

Implement Customer Service Guarantee and resolve service failures.

Develop effective and professional working relationships internally and externally promoting a positive image of the service and of Clarion.

Plan and organise own work and follow office procedures and administrative systems, including keeping service users’ files up to date.

Meet key performance targets in relation to Support Plans, Risk Assessments, Rent Arrears (with SHM) Customer Service Guarantee, and provide monitoring and other reports as required.

Keep your professional knowledge updated e.g. of legislation, welfare rights, procedures, resources available for service users.

Contribute to ensuring value for money and budgetary control by:

Keeping records of expenditure
Following procedures for purchasing including petty cash and banking
