* To conduct a variety of clinical assessments and reviews for
service users that are proportionate to need and be able to
interpret them to provide the appropriate care for each

* To identify risks to the individual and respond appropriately thus
ensuring any safeguarding issues are addressed.

* Enable individuals to identify appropriate outcomes which will
meet their needs and aspirations and work with them to achieve
these, through an agreed support plan.

* To raise the standard of professional work within the team by
contributing to training initiative and core training courses.

* To ensure performance and client contact information is recorded
and maintained accurately and comprehensively using the
appropriate systems in accordance with approved procedures. In
addition to ensure that services users have the required
information about their assessed need and agreed support plan and
ensure that all correspondence meets with corporate guidelines.

* To help develop and implement strategies for the delivery of
personalisation, self directed support and well being and
continually find new ways of promoting independence, incorporating
these into routine practice and applying the principles of

* To maintain an awareness and keep updated of changes in related
theories, current legislation, corporate and directorate policies,
local and agency practice in order to disseminate knowledge and
contribute to the delivery of a high standard of service and best.

* To ensure that predefined targets and outcomes are met keeping the
Practice Manager informed of progress.

* To manage a high, diverse and complex number of cases within the
parameters of agreed policies and practice together with the
professional guidance and support from the Practice Manager.

* To ensure that the social work practiced encompasses the
principles of Dignity in Care and that the service users are
treated with respect in all aspects of the service they receive.

* To take responsibility for ensuring that the Promoting
Independence Commitment Statement is signed and is personally
committed to in order that it is identified as part of the
performance framework for this post.

* To communicate both orally and in writing in a confident and
assertive manner in line with established policies, practices and
priorities of the Council.

* To work with the social work teams to continually develop and
consolidate knowledge base and level of skills enabling the
assignment of more complex caseloads to meet the ever changing
needs of service users.

* To optimise opportunities for users/carers to seek out alternative
solutions to Council funded care.

* To have a sound knowledge of performance indicators, targets and
standards and to perform to a high standard within a challenging
performance management culture.

* To enable the service user to identify outcomes and undertake
un/assisted support planning. Where appropriate, present ideas and
solutions as to how these could be achieved including identifying
contingency arrangements to cover any unexpected circumstances.

* To facilitate support planning by providing service users with the
fullest possible information about the range of support options
available, including signposting to increase well-being and
following up with the individual to ensure that the information
provided was satisfactory.


* Professional Social Work qualification, such as DipSW or CQSW and
registration as a Social Worker with the General Social Care

* Experience of working with adult client groups or similar field
involved in assessment of needs, care/support planning and case

* Experience of working with people from ethnic minority communities
and from inner city environments
