Purpose of Role: To act as a Leaving Care Personal Advisor working with young people leaving care, from the ages of 18-21 (24 if in full time education) towards a smooth transition into independence. Main Areas of Responsibility To hold a case load of young people leaving care ages 18-21-24 years. Engage with and provide support for young people, including those with significant additional needs to identify and address barriers to personal development, learning and work, particularly during periods of transition. Assess young people’s needs and act as lead professional, developing clear action plans and involving specialist services where appropriate. Engage effectively in multi-agency working developing collaborative relationships with relevant staff in a range of agencies to broker and co-ordinate support for young people. Where possible, promoting that young people keep contact with their significant others, including family members/carers and others, to enable them to contribute to maximising the young person’s potential. To provide direct support to young people preparing to leave care in order to ensure a smooth and positive transition to independent living. To keep up to date with national and local policy, legislation, standards and guidance in relation to Asylum, Social Care, Housing and Leaving Care. To support care leavers who have complex and multiple needs. To co-ordinate holistic support packages in order to meet their complex needs e.g. Mental Health, Sexual Abusers, Chronic Drug & Alcohol abuse. The duties and responsibilities listed above describe the post as it is at present. The post holder is expected to accept any reasonable alterations that may from time to time be necessary You will need Effective skills in organisation, time management and in managing a complex workload. Ability to communicate effectively (in both written and verbal forms) with young people, their families and other professionals. Knowledge of child development, adolescence, social, emotional and mental health needs. Ability to use structured assessment tools and plan intervention. Ability to engage and safeguard young people and families. Ability to work in partnership including voluntary sector. Ability to engage with young people who may be parents. A full driving licence DBS This post is classed as having a high degree of contact with children or vulnerable adults and is exempt from the Rehabilitation of Offenders Act 1974.An enhanced disclosure will be sort through the Criminal Records Bureau as part of pre-employment checks.
