We are looking for a Band 7 Team Lead to join our Falls Prevention Team at Gravesham Community Hospital. ​Candidates with AHP or Nursing backgrounds will both be considered for this role.

You will provide holistic assessment, diagnosis and treatment for patients presenting with a range of medical, surgical, and neurological conditions whilst managing and supervising the other staff members in the team. 

You'll be based at Gravesham Community Hospital, with the expectation to attend patient home visits, and associated sites within HCRG care group and across partnering organisations
This is a weekday service operating within the hours of 8:30 to 16:30.

Main ResponsibilitiesAs the Band 7 Team Lead you will be responsible for but not limited too:


Providing comprehensive assessment, diagnosis, advice, and treatment as appropriate, to all patients referred to the service requiring a falls assessment and/ or intervention.
Providing patients with the identified support and aids prior to discharge from the team, to facilitate the promotion of independent living within their chosen environment.
Educating and training members of the multi-professional team, other professionals, and carers on the therapeutic management of patients referred into the service.
Providing expert advice for patients with complex needs to other community-based services as appropriate. E.g., District Nursing, Social Services.
Undertaking evidence-based projects, research, and audit under the guidance of the Senior Management team
Leading, managing and supervising the Falls Team on a day to day basis, providing clinical leadership and management support and guidance.

The Ideal CandidateYou will have a degree in Physiotherapy, Occupational Therapy or Nursing. Holding a current HCPC / NMC Registration. Having 2 years specialist Band 6 experience including supervising Band 5, Band 4 and Band 3 staff in the team.

You should have knowledge and understanding of NICE guidelines as well as up to date knowledge of recent research to inform evidence-based practice.

Having an MSc degree is advantageous; candidates willing to undertake post-graduate degree will be considered also.
