A fantastic opportunity has arisen for a full-time/part time/job share Occupational Therapist/s to work in our specialist housing adaptations team. 
 
About HEART (Home Environment Assessment and Response Team) 
 
HEART Occupational Therapists and Housing Assessment Officers assess customers’ needs and housing conditions and offer solutions on a range of issues including provision of equipment, minor adaptations, and applications for Disabled Facilities Grants (DFG) for major adaptations. 
 
HEART is a partnership service across the five district councils in Warwickshire and is committed to providing a high-quality customer experience by delivering the right solution at the right time. HEARTs overall aim is to ensure that people can maintain an independent life and ensure their home environment is adapted appropriately and is accessible, safe and warm.  
 
About the role 
 
This role is based in, but you will be able to complete agile working and work from home. You will be expected to be able to cover the whole of Warwickshire dependant on demand. You will be responsible for a case
load, completing assessments (remote and face to face) and working with customers to plan and deliver the support they need to remain at home safely and independently. 
 
You will have the opportunity to  
 

Develop specialist skills and knowledge in housing adaptations.
Work alongside technical officers, contractors and architects. 
See a case through from assessment to finished adaptation ensuring that the customer has a seamless service and is at the heart of the process. 

You will be supported through regular clinical supervisions, and benefit from a team commitment towards Continued Professional Development.  
Key Requirements

A diploma or degree in Occupational Therapy.
Registration as an Occupational Therapist with HCPC
 A keen interest in housing adaptations and Disabled Facilities Grants and a willingness to learn. 

Our current vacancies are shared between the North and South HEART team.
