Working closely with our Community Nursing Teams, as a Healthcare Assistant you will support and assist the senior team members to provide care to people in their own homes for a wide range of conditions including diabetes, wound and end of life care. Our work is crucial to the community as we aim to prevent unnecessary hospital admission.

As part of our community nursing team we empower our staff to work autonomously and with flexibility by providing them with state of the art new tablets for them to be able to check in from home, write their notes up on the go and allow them to not be trapped in an office base, so they can really enjoy working in their chosen community background!

You will be working in a part-time, permanent role, working flexible hours and 1 in 4 weekends. You will have the opportunity to work in a multidisciplinary, supportive and collaborative team.

Main ResponsibilitiesWithin this role, you will:  


Assist in providing nursing care as delegated by the Team Leader within patient’s homes.
Maintain knowledge, skills and attitudes necessary to deliver a quality service that is responsive to the changing needs of patients.
Provide clerical and administrative support to the team as delegated by the Senior Sister/Team Sister
Deliver care in accordance with organisation policies, procedures and guidelines, according to competencies
Report any changes in condition of the patient to the Team Leader or in their absence the designated nurse in charge of the caseload. 

The Ideal CandidateThis is an opportunity to put your NVQ level 2/3 or equivalent/appropriate qualification/experience to great use. With strong communication and organisation skills, and a flexible approach will ensure you deliver positive experiences to patients at every stage of their healthcare journey. 

In this role, it is essential you have a driving license, allowing you to travel to service users' homes within the area of West Lancashire. 

You will also need to be confident using IT systems and be able to demonstrate working knowledge of Microsoft Office packages i.e. Outlook, Word, Excel.
