We are looking for a Full Time Healthy Family Practitioner to join our friendly Lancashire 0-19 Health Visiting Team working 30 hours per week..
As a Healthy Family Practitioner you’ll join a team who are focused on having a positive impact on children and families by supporting those in your local community through facilitating parenting groups and offering 1:1 guidance to really make our service users and staff feel the difference.
This service is due to transfer to Wirral Community Health Care NHS Foundation Trust (WCHC) later this year.

Main ResponsibilitiesAs a Healthy Family Practitioner within HCRG Care Group you’ll join a team who are focused on clinical excellence and positive outcomes for children and families.
Your days will be rewarding and varied and include:
To undertake mandated health reviews as delegated, for example Pre 1 year and 2- 2.5year Developmental checks. Identify children who may need further support and refer into specialist services as required
To provide targeted support for children and families in line with your competencies, as delegated by Health Visitor, School Nurse or Staff Nurse, for example behaviour management, infant feeding and other delegated packages of Care
Working with parents and families to help the children reach their potential and to really make our service users and staff feel the difference

The Ideal CandidateWe are looking for someone who can work as part of an effective team, who is able to work flexibly across Lancashire. You must be passionate about the wellbeing and development of children in the local area.
We are looking for something who has
A good general education and evidence of personal and professional development.
NNEB qualification, or equivalent to BTEC (National Diploma in Nursery Nursing) or equivalent such as NVQ level 3 (Childcare and Education) or Foundation Degree in Early Years/Education
Full UK driving license and access to a vehicle
