We’re looking for an enthusiastic and passionate Band 5 Community Nurse to join our friendly and dynamic Community Neuro Rehab Team based in Gravesend, North Kent to help us to continue to deliver care to patients with a neurological condition and change things for the better.

Gravesham Community Hospital is located a short distance from St Georges Shopping Centre and is surrounded by well-known food establishments and local coffee shops making it an ideal location to be based from.

This is a full time (37.5 hours per week) opportunity. 

A full UK driving licence and access to a vehicle for work purposes is essential.

Main ResponsibilitiesKey responsibilities include:


Participate in assessment and care planning of patient care in conjunction with the Senior Nurse
To work as part of the multi-disciplinary team.
To take a lead in using and supporting evidence based guidelines across the Specialist Community Neurorehabilitation Service
To act as a source of specialist nursing knowledge and provide support and advice in the field of complex neuro-rehabilitation.
To work closely with the MS specialist nurse and clinical specialist therapists within Dartford, Gravesend and Swanley.
To work as a member of the multi-disciplinary team within Specialist Community Neurorehabilitation Service ensuring that appropriate patients receive nursing and advice.
To have a holistic view of the different pathways within the Specialist Community Neurorehabilitation Service, making sure that all patients receive appropriate nursing support and advice.
To work closely with the local GPs, Hospital Consultants, hospital and community multi-disciplinary teams, and District Nurses.

The Ideal CandidateThe ideal candidate will have / will be:


1st Level Nurse Registration (registered with the NMC)
Evidence of continuing professional development
Mentorship/teaching qualification (within first 2 years of post)
Knowledge of recent developments within health and social care
Keeps up to date with evidence-based practice and research related to nursing and health care in the community
Ability to operate community equipment


You will also need to be confident using IT systems and be able to demonstrate working knowledge of Microsoft Office packages i.e. Outlook, Word, Excel.

A full UK driving licence and access to a vehicle for work purposes is essential.
