We have an exciting opportunity within our emerging Intermediate Care Team for a skilled and dedicated Band 6 Occupational Therapist to work in the Dartford, Gravesham, Swanley and Swale areas of Kent with the responsibility to provide specialist assessment, diagnosis and therapeutic interventions for adult patients living in the community with complex and complicated care needs, referred into the service.

In addition, you'll provide advice, support and education for patients and their families/carers, voluntary agencies and all those working with the patient regarding their medical condition and how this affects the person’s lifestyle and environment in order to maintain relationships, roles and independence.

This is a part-time position working 34 hours per week. There is some flexibility on how the 34 hours can be worked across Monday - Friday. A full UK driving license and access to a vehicle for work purposes is essential for this position

Main ResponsibilitiesAs a Band 6 Occupational Therapist you will:


Maintain the high standards of the department by contributing towards individual and service objectives.
Provide clinical support and supervision to junior colleagues and offer training to students.
Be able to work independently and as part of a Team, and to participate in innovation and service development of the pulmonary rehab service to benefit patients and colleagues.
Complete complex holistic assessments in the management of patients with chronic respiratory conditions.

The Ideal CandidateAs a Band 6 Occupational Therapist you will:


Have a HCPC registration 
Have a BSc/Diploma in Occupational Therapy   
Have knowledge of relevant national guidance and other relevant initiatives 
Have the ability to work in multi-disciplinary teams across health, social and voluntary sectors 
Have excellent communication skills 
Have motivation, adaptability and innovation skills
Need to be confident using IT systems and be able to demonstrate working knowledge of Microsoft Office packages i.e. Outlook, Word, Excel.


A full UK driving license and access to a vehicle for work purposes is essential for this position as it's Community based and travel across Dartford, Gravesham, Swanley and Swale areas is required.
