Position: Locum Independent Prescriber Pharmacist (Maternity Leave Cover)
Start Date: ASAP
Duration: Up to 12 months
Working Hours:
* 8:00 am - 4:00 pm
* 9:00 am - 5:00 pm
* 10:00 am - 6:00 pm
(Part-time or full-time - 2, 3, 4, or 5 days per week)
* Pay Rate: �30-�40 per hour (Ltd company)
* On-site: Must be on-site (No remote working)
* Weekdays only (no weekends/evenings)
Role Overview:
We are looking for a Locum Independent Prescriber Pharmacist to join our healthcare team in Wycombe. This is a maternity leave cover position, offering flexibility in working hours (full-time or part-time) and is expected to last up to 12 months . You will play a key role in the Long-Term Conditions (LTC) team , providing comprehensive reviews and ongoing management of patients with chronic diseases.
Key Responsibilities:
* Conduct holistic reviews of patients with long-term conditions such as:
* Hypertension
* Diabetes
* COPD
* Asthma
* Lipid control
* Chronic kidney disease
* Heart failure
* Prescribe and manage treatment plans as an independent prescriber.
* Collaborate with the multidisciplinary healthcare team to deliver high-quality patient care.
* Provide expert advice on medication and treatment to patients with chronic conditions.
* Ensure compliance with clinical guidelines and best practices.
Essential Qualifications & Experience:
* Independent Prescriber qualification (IP).
* Experience working in a pharmacy setting, ideally in a GP surgery or primary care.
* Strong knowledge of chronic disease management.
* Excellent communication and patient care skills.
* Ability to work independently and as part of a team.
