Do you have experience or a special interest in diabetes dietetics?Are you a team player who works with others to deliver a high-quality service, embrace innovation and would like the opportunity of variety that community settings offer?We are looking for a qualified dietitian who is passionate about supporting the delivery of an effective, responsive and caring service.This post is permanent, flexible (hybrid working), we are recruiting for a full time 37.5 hours dietitian and a part time 18.75 hours dietitian.Our service run’s between 8:30-4:30 Monday to Friday. The Diabetes Team currently consists of admin/diabetes facilitators/specialist diabetes nurses, specialist diabetes dietitians, pharmacists and GPi. We pride ourselves in supporting each other through peer support and clinical lead.A Specialist diabetes dietitian you will offer evidence-based dietary advice to patients with diabetes while considering factors including nutritional status, medication, diabetes control and lifestyle.Duties relating to your role include:MDT working with Diabetes Specialist Nurse, pharmacists and GPsi within the teamSpecialist Diabetes Dietetic clinics both in person at designated Leeds Community Healthcare health centres and via telephoneHome visits for patients living with Type 2 that are houseboundEducation groups run for people with Type 2 DiabetesResource/leaflet development for people with Type 2 diabetesStudent trainingAll staff are provided with a laptop and smart phone for hybrid working and remote access to SystmOne.If you have further questions regarding this role, please contactClinical Team Manager: Megan NippersEmail:m.nippers@nhs.netTelephone: 07846919856Interviews planned for the 17th of DecemberYou will work with adults who are living with Type II diabetes. The list below provides a detailed overview of your daily/monthly work activities.You will work as part of multidisciplinary team providing using systmOne to communicate with our wider MDT.You may be involved in audits and obtaining user feedback on service development. Examples include auditing our newly developed dietetic offer, collecting TOM’s and Friends & Family Test (FFT’s) to ensure effective patient care and accountabilityYou will offer 1:1 dietetic support for patients living with Type II diabetes who also require support with undernutrition, poor wound healing, weight management, enteral nutrition and finally patients with a hba1c > 70 who are undertaking a specific diet that may increase risk of hypoglycaemia/nutrition deficiencies. You will also support our nurses with patients on specific insulin regimes where diet may be impacting on hba1c levels. Examples of this include Basal Bolus patients and patients who are suffering from multiple hypo’s due to variable/erratic carbohydrate intake.To provide relevant information and counselling regarding diet in association with the individual’s ethical, cultural, religious, socio-economic and educational needs in relation to diabetes.To utilise highly developed behavioural management techniques with a focus on patient self-management. To attend relevant Team meetings, and other meetings as necessary to discuss care of patients and service provision. We current facilitate a monthly MDT and clinical supervision. This occurs on a Thursday.You will be involved in the development and distribution of appropriate evidence based dietary literature for patients to advise on specific dietary issues.To provide home visits for housebound patients where required following Trust home visit policyTo liaise with the Diabetes Dietetic Team Leader, Clinical Team Leader & Professional Lead for Dietetics and other Dietitians to provide a comprehensive diabetes service.Skills and QualitiesHCPC registered dietitian.Experience working in a multidisciplinary team.Experience or special interest in Diabetes.Car driver due to the service being citywide and access for home visits.Full training will be provided to allow you to manage a caseload of Type II diabetes patients. We have recently developed as an example Insulin competency which will allow our clinicians to manage, assess and treat patients on insulin regimes such as basal bolus.
