Job Advert: Band 6 Care Co-ordinator (CHMT) - Humber Area

Are you a passionate and experienced mental health professional looking for your next challenge? We have an exciting opportunity for a Band 6 Care Co-ordinator to join our Community Mental Health Team (CHMT) in the Humber area.

About the Role:
As a Band 6 Care Co-ordinator, you will play a pivotal role in providing high-quality, person-centred care to individuals with complex mental health needs. You will manage a caseload, working collaboratively with service users, families, and other professionals to ensure effective care planning and risk management.

Key Details:

* Pay Rate: �25-�28 per hour, depending on experience.
* Hours: Full-Time, Monday to Friday, 9:00 AM - 5:00 PM (On-Site Only).
* Location: Humber Area - Free Parking Available.
* Start Date: December 2024 - January 2025.
* Contract Length: 3-6 months, ongoing.

Key Responsibilities:

* Developing and implementing comprehensive care plans.
* Conducting risk assessments and ensuring appropriate interventions are in place.
* Coordinating with multidisciplinary teams to provide seamless and integrated care.
* Supporting service users in achieving their recovery goals and maintaining their mental well-being.
* Ensuring timely documentation and adherence to NHS guidelines.

Requirements:

* Professional Qualification as a Registered Mental Health Nurse (RMN), Social Worker, or Occupational Therapist (OT).
* Experience in Mental Health: A proven track record in community mental health services.
* Skills: Strong organizational, communication, and interpersonal skills.
* Registration: Current registration with the NMC, HCPC, or relevant professional body.
