Working Hours: Full-Time, Monday to Friday, 09:00 AM - 5:00 PM
Location: Birmingham
Work Arrangement: Remote working available. The team will provide IT equipment from Day 1 to ensure a seamless setup for home working.
An exciting opportunity has arisen for an experienced Band 6 Continuing Healthcare (CHC) Nurse to join a dynamic team within the NHS in Birmingham. This role is offered on a 3-6 month ongoing contract, providing full-time hours with a competitive pay rate of �40 per hour.
Key Responsibilities:
* Conduct comprehensive assessments of patients eligible for Continuing Healthcare (CHC) funding.
* Complete robust and accurate Decision Support Tools (DSTs) in line with the National Framework for NHS Continuing Healthcare.
* Liaise with multidisciplinary teams, patients, and their families to ensure person-centered care plans are delivered.
* Review and monitor care packages to ensure they meet the ongoing needs of patients.
* Provide expert advice and guidance on Continuing Healthcare processes and eligibility criteria.
* Ensure compliance with national guidelines and maintain accurate documentation.
Essential Requirements:
* Registered Nurse with valid NMC registration (Adult, Mental Health, or Learning Disability).
* Significant experience in Continuing Healthcare, including CHC assessments and completion of DSTs.
* Strong understanding of the National Framework for NHS Continuing Healthcare and NHS-funded nursing care.
* Excellent communication and interpersonal skills, with the ability to build positive working relationships with patients, families, and professionals.
* Ability to work independently and manage a busy caseload effectively.
