Our mission is to empower individuals to rebuild their lives and achieve independence.The service you will work in….Age Well Services are a key part of Wandsworth Council's approach to transforming adult social care and managing demand by enabling older people to retain, gain or regain independence; targeting preventative resources most effectively and ensuring value for money.At the heart of this approach is a vibrant voluntary and community sector (VCS) working in partnership with the Council, the NHS and other stakeholders (including businesses and faith groups) to help residents' access local services that enable them to improve or maintain their overall wellbeing and stay independent for longer.This is an exciting opportunity to be part of the open access Age Well Roehampton Service, with a focus on using existing assets to support older residents in a range of locations including community venues This service builds on the growing evidence that older people are enabled to contribute to and receive support from their community when they live in age friendly communities that are designed to encourage physical activity and healthy eating and bring people of all ages togetherA day in this role looks like….As a Coordinator at Hestia, you will play a crucial role in our Older People Service. Your contributions will directly influence the lives of our service users, guiding them towards a path of empowerment and independence.A day will never look the same and some of the things you can expect to do/be responsible for are:Work with the Team Leader to reach older people who may be at risk of losing their independence and offer support to either aid recovery or delay deterioration in their health & wellbeing.Actively engage and work in partnership with statutory, voluntary, and independent sector services, and residents aged 60 plus themselves, to optimise local resources and local knowledge in developing service provision.Support older people from Roehampton to use health and wellbeing services and link them into their wider communitiesContinue to develop Age Well services by engaging in provider forums and sharing best practice and expertise.With the support of Hestia's Fundraising Team and the Team Leader, explore opportunities to leverage additional funding and resources from other sources.Coproduce programmes of activity to support the fostering of community agency by enabling community members to come together to create change and realise a sense of control in individuals to help improve their health.Support older people and other residents to initiate and run their own groups close to their homes based around the interests of their members.Recruit and supervise volunteers, making best use of volunteers across the programme.At times, outreach into people's homes and, where appropriate into hospitals for regular service users who are in hospital for extended periods of time.Explore ways to embed digital solutions into Health and Wellbeing Services as appropriate.Deliver pee
