HCRG Care Group is seeking an enthusiastic and motivated Specialist Health Visitor to join our Vulnerable Families Team in Slough. This is a fantastic opportunity for a dedicated professional who is passionate about delivering outstanding specialist clinical services to a diverse and highly mobile population.

As a Specialist Health Visitor, you will play a crucial role in supporting families with complex needs, including those experiencing social deprivation, housing instability, or high levels of morbidity. You will hold a caseload of families receiving specialist and targeted services under the Healthy Child Programme, working in partnership with those facing challenges such as: 


New to the country
Drug and/or alcohol misuse
Domestic abuse
Mental illness
Temporary accommodation & disengagement from local services. 


Your expertise will be key in identifying highly vulnerable families, conducting assessments, coordinating family health plans, and ensuring targeted support to reduce health inequalities and improve long-term outcomes.

Main ResponsibilitiesAs a Specialist Health Visitor, you will be responsible for:


Specialist Support & Assessment: Access and apply research, legislation, and policy on homelessness, mental health, domestic abuse, and substance misuse to assess and support vulnerable families.
Multi-Agency Collaboration: Manage referrals from agencies such as midwifery, police, early help, and children’s services, contributing to audits, reports, and service planning.
Community & Outreach Work: Provide health and wellbeing support to families in temporary accommodations, including hotels, B&Bs, and refuges. Assess health needs and deliver targeted care in line with the Healthy Child Programme.
Language & Accessibility Support: Arrange translation services to help non-English-speaking families access healthcare and support.
Pathway Development: Establish and maintain links with health partners, primary care, local authorities, and voluntary agencies, particularly for new arrivals to the UK.
Monitoring & Evaluation: Record and track intervention outcomes to demonstrate service impact and effectiveness.
Representation & Training: Represent Slough Child and Family Wellbeing at panels (Early Help, MARAC, MASH, Housing) and contribute to multi-agency training and planning.
Professional Standards: Maintain accurate records, participate in supervision and audits, comply with clinical governance, and complete all mandatory training.

The Ideal CandidateEssential


Registered Nurse with a Specialist Community Public Health Nurse qualification (Degree/Diploma).
Additional post-registration clinical skills training relevant to the role (e.g., domestic abuse, substance misuse).
Strong safeguarding knowledge, with experience producing high-quality reports for multi-agency child protection teams and court proceedings.
Minimum two years' post-qualification experience, including working with vulnerable and hard-to-reach groups.
Proven ability to manage a complex caseload independently, supporting families with high levels of vulnerability.
Expert knowledge in child and family health and development.
Advanced communication and engagement skills, demonstrating resilience in challenging situations.
A strong commitment to equality, diversity, and anti-discriminatory practice.
Excellent observation and listening skills, with a compassionate and professional approach.
Ability to work effectively under pressure, multitask, and maintain accuracy in data recording.
Proactive in identifying and escalating concerns to ensure early intervention.
A flexible and adaptable mindset, with a commitment to continuous learning.


Desirable


Further educational development, such as a Master’s degree or postgraduate training in a relevant field.
Leadership training and/or experience in line management.
