What You'll Be Doing:
Join our Dementia Team as a Band 6 CPN and provide specialist mental health support to individuals living with dementia. Your responsibilities will include:
Conducting comprehensive mental health and cognitive assessments for individuals with dementia.
Developing and implementing personalised care plans to support patients and their families.
Providing specialist interventions to manage challenging behaviors and cognitive decline.
Supporting patients with medication management and monitoring side effects.
Offering crisis intervention and risk management strategies to ensure patient safety.
Liaising with families, social workers, GPs, and other healthcare professionals to provide holistic care.
Maintaining accurate and up-to-date patient records in line with NHS standards and guidelines.
Requirements & Qualifications:
To be successful in this role, you will need:
A current and valid NMC registration as a Mental Health Nurse.
Previous experience working with dementia patients in a mental health setting.
Strong communication and interpersonal skills.
The ability to work independently and within a multidisciplinary team.
A commitment to delivering high-quality, patient-centered care.
