What is the job role?The service will operate across 7 days, Monday – Friday 8.00am – 8.00 pm and Weekends 8.00am  – 4.00 pm for which a shift allowance of 10% is paid. We have Social Work posts within our Hospital Community Discharge team. You will be supporting people living with a wide range of social care issues. There is a real emphasis on promoting independence and supporting safe and timely Hospital DischargeOur teams embrace strength-based practice, support Making Safeguarding Personal and work within key legislation such as the Care Act 2014 and the Mental Capacity Act 2005. You will need to have the confidence to advocate for clients and work in partnership with health colleagues’ other statutory partners and voluntary sector providers.  As a practitioner you will need to have the ability to be efficient, creative, innovative and search for solutions that are not always obvious.
