Our mission is to empower individuals to rebuild their lives and achieve independence.The service you will work in….At Lewisham Mental Health Service, we provide supported accommodation for single adults with complex mental health needs who are at various stages of their recovery. Our service is part of the London Borough of Lewisham's Mental Health Supported Housing Pathway, which promotes recovery and independence to individuals with a mental health diagnosis.The service is dedicated to helping service users develop the skills and networks needed to maximise their independent living abilities and transition to suitable accommodation, whether it be alternative supported housing or independent living.A primary focus of our service is to enhance engagement with mental health treatment. Therefore, it is crucial for us to collaborate effectively with key agencies, such as SLaM, to improve the mental health and wellbeing of our service users.The service consists of 2 x 24-hour accommodation within the borough of Lewisham. Tredown Road has capacity for 8 service users and Coulgate Street has a capacity of 7. At the moment both sites are male only, however depending on need this could change to mixed accommodation. A day in this role looks like….As a Support Worker at Hestia, you will play a crucial role in our high support mental health service. Your contributions will directly influence the lives of our service users, guiding them towards a path of empowerment and independence.A day will never look the same and some of the things you can expect to do/be responsible for are:To engage service users in regular and comprehensive assessment, support planning and safety planning processes, ensuring that all plans and assessments are person-centred and user-led.To work with the Service Manager/Team Manager to assess referrals, formulate and review support plans, safety management plans and move on plans in partnership with all service users.To work in partnership with all appropriate external agencies involved in the support of service users.To work in partnership with families, careers and other key supporters of the service users around their support and recovery plans.To ensure that service users are able to attend and participate in a range of socially inclusive activities and skills development opportunities.To advise on and broker access to health services, education, training and employment opportunities related to the needs and aspirations of each individual within the service in conjunction with the Specialist Workers.To work flexibly as part of the team to ensure service coverage and that contractual obligations are met, including being part of a rota.To ensure the environment is effective in promoting recovery, including health and safety checks and reporting and the follow up on repairs and maintenance.To support and enforce service user adherence to the terms of their occupancy agreement.
