An exciting new opportunity has arisen to work as a specialist diabetes pharmacist, in an innovative and forward thinking PCN.Woodsley and Holt Park PCN are committed to developing a multidisciplinary Diabetes Support Team (DiaST) to support with the care of people living with diabetes and those who have been newly diagnosed. We are looking for a pharmacist who has specialist experience in diabetes to join a team of dieticians, health and well-being coaches and a podiatrist, to improve the care of people mainly (but not exclusively) with type 2 diabetes.This exciting new role will give you the opportunity to develop your specialist clinical skills in a patient-facing, primary care environment, working with a wide variety of healthcare professionals.
