As a ‘Health Improvement Practitioner’ you will work in our BaNES Integrated Neighbourhood team and be a source of specialist advice and support for our XPert diabetes service for some of the most vulnerable people living in Bath and Northeast Somerset. Although your main focus will be supporting people to manage their diabetes diagnosis, you will at times to a lesser extent, support people with weight management, healthier cooking and physical activity in-line with our person-centered approach to improving peoples’ overall health and wellbeing. This work will be carried out on a one-to-one and group basis within different settings throughout Bath and Northeast Somerset..As a 'Health Improvement Practitioner,' you will work 22.5 hours per week where your working hours will be spread over 3 days FTC until March 31st 2026.. You'll provide structured support to individuals living with diabetes through a range of settings including group sessions, one-to-one support, telephone and digital engagement. A key part of the role involves delivering training to health professionals and promoting the programme to increase awareness and accessibility.You'll work closely with local services, particularly secondary care, to ensure continuity of care for those with diabetes.
